Community Health Connections (CHC) is a local nonprofit, federally qualified health center serving north central Massachusetts. CHC serves all, regardless of health insurance status or income. Services include Medical, Dental, Behavioral Health, Substance Use Disorder, Podiatry, Optometry, Pediatrics, Urgent Care and Specialty Services. CHC is proud to serve the residents of the Greater Gardner area with two locations: 175 Connors Street & 529 Timpany Boulevard (opening June 2023)
