Case Manager

(Internal Title): Housing Retention Case Manager

This is a full-time benefitted position. Hours are Monday-Friday 9am-5pm with some evenings and weekends required. This is an in person position headquartered in Gardner, MA that requires travel throughout North Central Mass and surrounding cities and towns.

Reporting to the Executive Director, the HRCM will work under the Housing Retention Grant as awarded by the federal US Department of Veterans Affairs.

Responsibilities:

  • Delivery of services to community veterans in alignment with housing retention protocol and procedures, managing an average caseload of 16 veterans. Eligible clients include veterans whose housing is insecure and veterans who have been recently rehoused within our North Central Massachusetts service delivery area
  • Collaborative work with case managers, outreach, counseling, and program personnel
  • Maintain compliance with Executive Office of Veterans Services (EOVS), Veterans Administration (VA), Department of Labor (DOL), and/or any other pre-requisite regulations and standards
  • Demonstrate proficiency in assessment and housing, maintain knowledge of community employment and training resources, and maintain referral networks throughout service delivery area
  • Ensure immediate basic needs of caseload approved clients are met
  • In coordination with clinical director and client, create service plans for caseload approved clients and follow through with identified goals
  • Increase veterans’ awareness of available resources from the VA, the state, and within their community
  • Prepare and submit all required reports as instructed by clinical director and associated VA staff
  • Ability to work and communicate effectively with clients and staff
  • Work within the guidelines and needs of the program administrator and clinical director
  • Support the efforts of other MVOC staff as requested
  • Maintain an orderly and professional working environment
  • Establish effective priorities and possess time management skills

Requirements:

  • Bachelor’s degree preferred or equivalent work experience required. Associate’s degree in human services or related field and/or experience working with veterans and families will be considered.
  • Exhibit ethical conduct, integrity, and effective interpersonal communication
  • Ability to work independently
  • Proficiency in Microsoft office and client database software
  • Valid drivers license
  • Veteran status strongly preferred

Pay Range: $50,000-60,000

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
